You need Create moderation cases.
Steps
Open Moderation (the Moderation Dashboard: "Manage cases, bans, and moderation actions").
Choose Create new case.
Target Username: type the player's Roblox username and choose Search. Look them up before you can create the case ("Look up a Roblox user above to enable Create Case"). The player does not need to be in your workspace.
Reason: a short, publicly visible reason. This is what the punishment shows. Keep it factual and professional.
Description: internal notes for moderators. Add as much detail as possible: what happened, where, when, who saw it.
Planned Action: the action you propose: None yet, Warning, Kick, Temp Ban or Perm Ban.
For a temporary ban, choose when it expires.
For a warning (in workspaces with the newer moderation features), you may set how many days the warning lasts ("Warning lasts (days)"). After that it is shown as ended and stops counting. Leave it empty for a warning that never ends.
Affected Places: choose specific games to restrict the action to, or leave it for all.
Choose Create Case.
Nothing is enforced yet. A case is a proposal until someone executes it (see "Execute, revoke or delete a punishment").
Find cases
The dashboard lists cases with their Target User, Reason, Status, Punishment, Evidence count, Created date and actions. Use search (by username or reason), the status filter and the page controls.
Status of a case
A case starts open. It becomes resolved when its action is carried out (a kick executed, a ban made) or when someone resolves it. A revoked case is one whose punishment was taken back.
Edit a case
You can change the reason, description, notes and planned action if you opened the case or have Edit moderation cases. Every change is written to the case's log.
Tips
Write the reason as if the player will read it, because they may.
Put names, times and evidence references in the internal description.
Add evidence straight away (see "Evidence: what you can attach to a case and how it is checked").
